Making an Appointment

If you are one of our future clients that already has been referred for services through Fresno County and would like to schedule an appointment, please contact our office at (559) 256-2000 between the hours of:
8:00am and 7:00pm Monday – Thursday
Fridays 8:00am -5:00pm

In order to speed up the scheduling process please have access to the client’s Medi-cal number, date of birth and social security number.

CPI understands your time is important, therefore our intake paperwork has been made available to you online. 

Authorization for the Request, Release and Exchange of Client's Health/ or  Mental Health Information

Please inform the office if you would prefer these documents mailed directly to you.

We ask that you review and complete intake documents prior to your appointment. Please be sure to bring all intake documents with you at the time of the assessment. If there are questions or concerns that have made it difficult to complete the intake packet, please plan to arrive 30 minutes prior to the appointment so that our staff may assist you.

At the time of your appointment you will need to bring in the following:

• The client’s Medi-Cal card and any other insurance information.
• Any and all court documents identifying you as the legal guardian.

We are happy to report that most assessments are scheduled within 10-14 days of referral.
